What to expect at your first appointment:
For all patients, the first visit is a consultation.  Please fill out the registration packet and bring it to your first appointment.  If you are unable to print or complete the forms, please arrive 30 minutes early for your first appointment.

Please bring the name and telephone number of your physician, as well as a recent list of medications, allergies, and surgeries.  We also recommend bringing a copy of any recent dental x-rays (within the past 2 years).

For suitable patients, an exam and x-rays are also completed at the first visit.

We are not an emergency clinic.  If you are a patient of record under the care of Dr. Grimaldi or Dr. Boychuk and need to speak to an on-call dentist outside of normal operating hours, please call the clinic line and follow the appropriate prompts to reach the after-hour call service.
